A 16th-century building located on a hill near the hamlet of San Rocco. Its construction dates back to between 1510 and 1520, preceded by a portico on the right leading to the cemetery built between 1810 and 1820.
The original building was erected by the municipality as a lazaretto against frequent plagues in a location isolated from the town. 
In front of the presbytery is the tombstone of the nobleman Paolo Parisio, commissioned by his daughter Camilla Asti in 1607. There is also a polychrome wooden statue of St. Rocco similar to the crucifix by Ludovico da Nozza from 1508, which can therefore be dated to 1520-1530 by an imitator. 
The church was managed by the Confraternity of St. Roch, founded in 1447. Originally, the church was officiated by the Confraternity of the Disciplini and housed the remains of Archpriest Febbrari from 1842 to 1907, when they were moved to St. Stephen's. Archpriest Antonio Racheli was buried in the empty tomb in 1917.
It housed an Immaculate Conception by Antonio Balestra (166-1740), now stolen, while the tabernacle on the right once contained the relics of Saints Rocco and Antonio da Padova.
